This is the Apple Health Macro Nutrients Tracker.
It keeps a library of added food in your iCloud Drive. This needs a modified version of the Shortcut “libStorage”, which can be found here:

The original version of libStorage was shared on r/shortcuts by user chilaxan.

Functionality should be pretty self explanatory. First choose a food from the Library, or add a new entry. Adding a new Food requires kCal, carbs, fat and protein per 100gram. To add a food consumption to Apple Health, the amount and date needs to be entered.
